- Completed Officer Candidate School Application
- Birth Certificate (Copy)
- SSN Card (Copy)
- Proof of Citizenship (If Applicable)(Must have before beginning Ph 3)
- Name Change Document (If Applicable)
- Chapter 2 Commissioning Physical from MEPS (To include DAT/HIV results)
- Age Waiver (for ages 30 to 39). At this time OCS will not accept applicants 42 years of age or over.
- (Physical will not be over 2 years old at time of commissioning). May start course with current Enlistment Physical; however, must have Commissioning Physical NLT 90 days after completion of Phase 1.
- College Transcript (Original not student copy) (Must have at least 60 SH shown on one transcript)(Must have 90 SH before beginning Ph 3)
- If you do not have a bachelors degree a degree completion plan must be obtained. This plan will identify the courses and date in which the bachelor degree will be obtained. this plan must be verified by the administration or a guidance counselor of the istitution you are attending or will attend.
- GT score (MIN 110) must be annotated on DA 2-1, enlistment contract, or retest memo.
- DD 214 (Certificate or release or discharge active duty) / DD 220 (Active duty report) / NGB 22 (Report of separation and record of service) reflecting all periods of service. All must indicate Honorable service. (Copy)
